Penny is also a governor at a large NHS hospital. There are many parallels to be drawn between aviation and other safety critical and highly regulated industries. The case of Daniel Pelka who suffered horrendous abuse and later died from neglect, made Penny realise that child protection could benefit from Human Factors training and Near Miss Reporting. Penny became a member of The Safer Safeguarding Group because she wanted to be able to make a difference to the lives of vulnerable children.

She truly believes that we can make children safer, we just need our campaign to be heard by those in a position to effect the cultural shift needed. A robust safety management system and a training not blaming philosophy would be a good place to start. Many of the failings have been due to a loss of awareness of the seriousness of the situation, high workload, a bullying culture within some departments, lack of communication, to name a few. Human Factor training would give front line staff the tools to perform more effectively and the confidence to voice their concerns, allowing others to learn from experiences reported.
